A Good Idea
Description
The Adult Day Health Care Centers help delay or prevent premature placement in a nursing home, psychiatric facility or other institution for the frail elderly and other physically or mentally impaired adults 18 and over. The ADHC is a dual-model program which provides a medical model and a social model. The emphasis of care will revolve around the medical model which promotes restoration or maintenance of physical and/or mental functioning ability and will monitor or provide for medically related needs of participants. The social model provides non-medical care, supervision, and guidance or training essential for sustaining the activities of daily living.
Goal / Mission
The ongoing goals of the ADHC include maintaining functional independence and preventing premature institutionalization while providing respite and a support system for caregivers and providing a therapeutic environment involving; physical , occupational, speech, recreation, psycho-social and creative arts therapy to increase the quality of life for participants.
Results / Accomplishments
Through the Centers, high-quality comprehensive care at a reasonable cost is provided to the community. The program will assist qualified participants by providing a support network as well as an opportunity to restore or maintain functional independence that is commensurate with existing and potential capabilities through a multi-disciplinary team approach.
